The Care Manager Role: A Key Player in Bright Health's Mission As a Care Manager - LPN/LVN (Bilingual Spanish) at Bright Health, you'll play a vital role in coordinating and facilitating members' individualized plans of care. You'll work closely with members, families, caregivers, and healthcare providers to determine needs, provide support and guidance, and act as a liaison between primary care physicians and specialty physicians. This 100% remote role requires a California resident and offers the opportunity to make a meaningful impact on the lives of our members. Key Responsibilities: Conduct face-to-face visits with members in healthcare facilities and their homes, and initiate telephone outreach to ensure comprehensive care. Facilitate 'Intakes' and maintain required documentation in our EMR web-based application, including surveys, health risk assessments, progress notes, and other documentation as required. Utilize the Transition of Care model and nursing process to assess, triage, plan, implement, and evaluate care that meets members' needs. Provide health maintenance, medication, and disease management education to members and families/caregivers, empowering them to take control of their health. Coordinate member care appointments, community resources, referrals, management program benefits, and special focus programs to ensure seamless care. Ensure members' preventive care appointments are scheduled and encourage participation to promote healthy outcomes. Maintain compliance with CMS-mandated health risk assessments, care plans, and interdisciplinary care team requirements. Participate in Interdisciplinary Care Team presentations, if indicated, to collaborate on member care. Coordinate continuity of care, prevention, and avoidance of complications, and close clinical treatment and management under the direction of primary healthcare providers and medical directors. Effectively communicate, problem-solve, and maintain productive relationships with members, families, caregivers, and healthcare providers. Work with outside facilities, agencies, and vendors to maintain positive working relationships and ensure comprehensive care. Support members and their families/caregivers, providing compassionate care and empathy. Make referrals/linkages to community resources on an individualized basis to address member needs. Essential Qualifications and Skills: To succeed as a Care Manager - LPN/LVN (Bilingual Spanish) at Bright Health, you'll need: A current Licensed Practical Nursing or Licensed Vocational Nurse certificate from an accredited Nursing Program. Two (2) or more years of experience as an LPN/LVN, preferably in a case management setting. A valid driver's license, auto insurance, and a safe driving history. Awareness and experience with HIPAA requirements for healthcare communications preferred. Bilingual (English/Spanish) skills are preferred. Excellent communication skills, both oral and written, to effectively interact with members, families, caregivers, and healthcare providers. The ability to approach challenges calmly and objectively, identifying the best solution in complex situations. A strong operational mindset, using data to draw insights and drive results.
