Develop and implement individualized care plans for members with behavioral health issues. Conduct thorough assessments to identify the needs and goals of each member. Collaborate with members, their families, and healthcare providers to coordinate and facilitate treatment and services. Monitor member’s progress and adjust care plans as needed to ensure optimal outcomes. Provide ongoing support and counseling to members to promote self-sufficiency and improved overall well-being. Connect members with appropriate community resources and support services. Maintain accurate and up-to-date documentation of member interactions and progress. Advocate for members and ensure their needs are being met within the healthcare system. Stay current with industry standards and best practices in behavioral health care. Communicate effectively with team members and other healthcare professionals to ensure coordinated and comprehensive care for members. Participate in regular team meetings and provide input on ways to improve the delivery of behavioral health services. Adhere to all confidentiality and privacy regulations in handling sensitive member information. Act as a liaison between members, their families, and healthcare providers to facilitate treatment and resolve any issues or concerns. Collaborate with insurance providers and other stakeholders to ensure appropriate coverage for members’ treatment. Participate in ongoing training and professional development opportunities to enhance job knowledge and skills. Maintain a positive and compassionate attitude towards members and their families, promoting a supportive and caring environment.
