Key Responsibilities Provide both clinical and administrative support to our care team, ensuring seamless patient interactions and high-quality care delivery. Manage incoming and outgoing patient communications, including phone calls and messages, with a focus on protocol-based triage and timely resolution. Prepare for patient visits by reviewing schedules, ensuring all necessary documents are available, and proactively obtaining missing information. Assist in the completion of various forms and reports, such as disability forms, home health orders, and disease status letters, under the guidance of our providers. Identify patients who are at risk for no-shows and engage in proactive outreach to prevent missed appointments and facilitate rescheduling when necessary. Support our care team in managing patient populations, recognizing vulnerable groups, and initiating appropriate social service referrals. Demonstrate proficiency in our Electronic Health Record System (EHRS), utilizing it to document patient interactions, manage phone calls, and maintain accurate records. Qualifications and Skills Essential Qualifications A high school diploma or equivalent is required; Medical Assistant Certification or equivalent experience is highly valued. At least 2 years of experience in an ambulatory or virtual clinical setting, with a background in pediatrics or a related field being a plus. Fluency in Spanish is mandatory, reflecting our commitment to serving diverse patient populations. Proven competency in an EHRS system, with the ability to adapt to our specific platform. Excellent communication and customer service skills, with a patient-centered approach. Preferred Qualifications Previous experience in a value-based care environment or a similar innovative healthcare model. Knowledge of pediatric care practices and the challenges associated with managing complex medical conditions in children. Familiarity with healthcare technology and telehealth platforms.
