You will work closely with our multidisciplinary care team to assess patient needs, develop and implement individualized care plans, and provide support and consultation to patients and their families. Your expertise will be essential in helping patients maintain the greatest level of independence and achieve their goals. **Key Responsibilities** * Provide appropriate social work interventions to patients and their families, including psychosocial assessments, brief individual or family counseling, advocacy, linkage, referral, group work, case coordination, and long-term care planning and placement. * Consult and coordinate with our multidisciplinary care team, including family conferences, care conferences, team meetings, and other collaborative efforts. * Assess patient and family needs for care and communicate effectively with other healthcare providers, collecting relevant data and evaluating psychosocial assets and deficits to inform care planning. * Evaluate patient and family needs through timely assessment, communication, and observation, modifying the plan of care as needed based on patient and care system response. **Essential Qualifications** * Minimum one (1) year of previous social work experience (waived for current KP Coalition employees). * Master's degree in Behavioral Science or Master's degree in Social Work. * Licensed Marriage and Family Therapist (Washington), Mental Health Counselor License (Washington), Licensed Advanced Social Worker License (Washington), or Licensed Independent Clinical Social Worker (Washington). **Preferred Qualifications** * Two (2) years of post-masters experience. * Care coordination experience in managed care systems, inpatient or outpatient settings, skilled nursing facilities, or long-term care facilities. * Knowledge of social case management, conflict resolution, and team building practices. * Familiarity with Microsoft Office and electronic charting systems. **Skills and Competencies** * Excellent written and verbal communication skills. * Demonstrated success as part of a multidisciplinary team and commitment to diversity. * Strong assessment and evaluation skills, with the ability to develop and implement individualized care plans. * Ability to work effectively with patients, families, and healthcare providers to achieve positive outcomes. * Strong problem-solving and critical thinking skills, with the ability to adapt to changing situations and priorities. **Career Growth Opportunities and Learning Benefits** As a social worker at Kaiser Permanente, you will have access to a range of career growth opportunities and learning benefits, including: * Professional development and training programs to enhance your skills and knowledge. * Opportunities for advancement and leadership within our organization. * Collaborative and supportive work environment, with a focus on teamwork and mutual respect. * Competitive compensation and benefits package, including medical, dental, and vision coverage, retirement savings plan, and paid time off. **Work Environment and Company Culture** Kaiser Permanente is committed to creating a work environment that is inclusive, respectful, and supportive of all employees.
