Job Overview: LPN/LVN for Rehabilitation Services We're looking for a skilled and compassionate LPN/LVN to provide high-quality nursing services, training, and support to individuals with a range of needs, including brain injury, intellectual and developmental disabilities, substance use disorders, and emotional or behavioral disabilities. Key Responsibilities Monitor the health conditions of individuals served, ensuring that quality services are consistently provided. Administer medications and maintain accurate documentation, while also overseeing and monitoring staff members who administer medications. Provide prescribed medical treatment as per physician's orders and deliver personal care services. Conduct assessments and implement healthcare services as part of each Individual Service Plan (ISP), responding promptly to any changes in condition. Offer training to staff members on critical topics such as universal precautions, infection control, and medication administration. Act as a liaison and advocate with other medical and professional service providers, coordinating services with external agencies to ensure consistent treatment for individuals receiving care. Provide educational training to individuals served on various topics, including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation. Essential Qualifications Associate’s Degree in Nursing or a related field, or a state certificate. A minimum of one year of experience working with the population served in a nursing capacity. Current LPN or LVN licensure in the state of residence. Valid driver's license, car registration, and auto insurance. Certifications as required by state licensing regulations, such as CPR/First Aid, HIV/AIDS certification, Bloodborne Pathogens (BBP), and ACT training. Excellent communication skills, with the ability to effectively interact with staff, individuals served, and their families. Preferred Qualifications and Skills While not mandatory, the following qualifications and skills are highly valued: Experience working in rehabilitation settings or with individuals with complex needs. Knowledge of current nursing practices and the ability to apply them in diverse settings. Strong organizational and time management skills, with the ability to prioritize tasks effectively.
