Job Title: Remote Assessor Reviewer RN (MUST RESIDE IN Colorado, Iowa or Oklahoma) Job ID: 244677 Employment Type: Long-Term Temporary (Contract-to-Hire Potential) Start Date: August 25, 2025 Duration: Initial 6-month contract with possible extension or hire Schedule: Monday-Friday, 9:00 AM - 5:00 PM MST (flexibility required) Pay Range: $70,000 - $85,000 annually Location(s): MUST RESIDE IN Colorado, Iowa or Oklahoma • Colorado (Preferred): Hybrid with travel to patient homes • Iowa or Oklahoma: Fully Remote Overview: Seeking Registered Nurses (RNs) to join a new, high-impact program supporting individuals with Intellectual and Developmental Disabilities (IDD) enrolled in Colorado Medicaid. In this role, you will conduct face-to-face and/or virtual clinical assessments of members to evaluate their medical, personal, and caregiver support needs. This is an opportunity to make a real difference for vulnerable populations while working within a collaborative, mission-driven environment. Key Responsibilities: • Conduct 2-3 hour assessments with IDD members and caregivers • Use skilled care acuity tools to assess needs related to: • Clinical/medical status • Personal care and daily living support • Caregiver assistance • Document findings thoroughly and accurately in internal systems • Collaborate with healthcare providers to ensure coordinated care • Mentor new assessors, conduct quality reviews, and participate in process improvements • Provide education and resources to members and their families Minimum Qualifications: • MUST RESIDE IN Colorado, Iowa or Oklahoma • Bachelor's Degree in Nursing (BSN) required • Active RN license in Colorado or compact state • 3+ years of nursing experience within the last 10 years (excluding clinicals) • Experience working with individuals with IDD • Strong understanding of clinical assessments and medical terminology • Comfortable conducting assessments via video and in-home visits Preferred Qualifications: • Experience with pediatric and geriatric IDD populations • Bilingual (Spanish) strongly preferred • Strong critical thinking and attention to detail • Ability to work independently while maintaining empathy and professionalism • Flexibility in schedule to accommodate patient needs Tools & Systems You'll Use: • Microsoft Office (Outlook, Teams, Word, Excel) • Salesforce, CSC/QNXT, JIVA • NICE inContact (Max Agent, WFM, Nexidia) • Compliance 360, Macess, Workday Why Join? • Help launch a new state contract focused on supporting underserved communities • Potential contract-to-hire opportunity for high performers • Make a meaningful impact through person-centered care coordination • Work in a supportive, collaborative, and flexible environment For California Applicants: We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO) , and the California Fair Chance Act (CFCA).
