Job Summary We're seeking a highly skilled and experienced Licensed Practical Nurse - LPN - LVN to work 100% remotely as a care coordinator. As a remote LPN, you will be responsible for managing a patient census, identifying and coordinating community resources, providing patient education, and performing medication management. Key Responsibilities Patient Census Management: Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care. Community Resource Coordination: Identify and coordinate community resources with patients that would benefit their care. Patient Education: Provide patient education and health literacy on the management of chronic conditions. Medication Management: Perform medication management, including identifying potential medication concerns, reconciliation, adherence, and coordinating refills. Service Delivery: Assist in ensuring timely delivery of services to patients, including Home Health, DME, Home Infusion, and other critical needs. Patient Communication: Resolve patients' questions and create an open dialogue to understand needs. Referral and Scheduling Management: Assist/manage referrals and appointment scheduling. Requirements To be successful in this role, you will need: Active Multi-State/Compact License: An active Multi-State/Compact License (LPN) (NLC) (LVN) is required. Technical Aptitude: Proficiency in Microsoft Office Suite is necessary. Excellent Communication Skills: Excellent written and verbal communication skills are essential. Experience: At least 3 years of direct patient-facing work experience is required. Preferred Qualifications We prefer candidates with: Epic Experience: Experience with Epic systems is a plus. Bilingual: Bilingual candidates are highly valued. Additional Single State Licensures: Additional single state licensures (LPN) are preferred.
