About Our Group Home Care Program Our group home care program provides a supportive and nurturing environment for adults who require personalized care and attention. Key Responsibilities Provide skilled nursing care to adult clients in a group home setting, including administering medications, monitoring health status, and performing various medical procedures. Develop and implement individualized care plans to meet the unique needs of each client. Collaborate with our interdisciplinary team to ensure comprehensive care and achieve client goals. Maintain accurate and detailed records of client care, including progress notes and other relevant documentation. Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination. Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments. Essential Qualifications To be considered for this role, you must possess: A current LPN license in good standing. Previous experience in a healthcare setting, preferably in home health care or a related field. Strong clinical skills, including the ability to assess client needs and develop effective care plans. Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals. Preferred Qualifications While not required, the following qualifications are highly desirable: Experience working with clients with complex medical needs, such as tracheostomy and ventilator management. Previous experience in a group home or residential care setting. Certifications in specialized areas, such as wound care or medication management.
