Job Overview We are seeking an experienced and compassionate Behavioral Health Care Advocate to join our team in Southwest Virginia. As a field-based advocate, you will work closely with Medicaid members, providing personalized support and guidance to help them navigate the healthcare system. Your role will be multifaceted, and you will have the opportunity to work in a variety of settings, including hospitals, community centers, and members' homes. You will serve as an educator, evaluator, service coordinator, and community resource researcher, working to ensure that our members receive the care and support they need to thrive. Key Responsibilities Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices, to provide personalized support and guidance Develop and implement individualized care plans to address the unique needs of each member, including condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels Collaborate with healthcare providers, community resources, and other stakeholders to ensure that members receive comprehensive and coordinated care Assist members with transitions from care facilities back to their homes, providing support and guidance to ensure a smooth and successful transition Utilize your knowledge of local and state community resources to connect members with the services and support they need to achieve their health goals Essential Qualifications To be successful in this role, you will need to possess a strong foundation in behavioral health, as well as excellent communication and interpersonal skills. The following qualifications are required: Licensed Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling; or Licensed Ph. D. Active, unrestricted, independent license in Virginia: LCSW, LMFT, LPC, or LP At least 1 year of experience working in behavioral health, with a focus on delivering personalized support to individuals with complex needs Proficient computer skills and good working knowledge of Microsoft Word and Outlook Access to reliable transportation that will allow you to travel within an assigned territory in Southwest Virginia Access to secure, high-speed internet (Broadband Cable, DSL, or Fiber) and a dedicated, distraction-free workspace at home Preferred Qualifications The following qualifications are preferred, but not required: At least 1 year of experience working with the Medicaid population, with a focus on delivering personalized support to individuals with complex needs At least 1 year of Community Mental Health experience, with a focus on delivering evidence-based interventions and supporting individuals with serious behavioral health needs At least 1 year of experience working with local and state community resources in Virginia, with a focus on connecting individuals with the services and support they need to achieve their health goals Medical/Behavioral setting experience (i.e., hospital, managed care organization, or joint medical/behavioral outpatient practice), with a focus on delivering comprehensive and coordinated care Dual diagnosis experience with mental health and substance abuse, with a focus on delivering evidence-based interventions and supporting individuals with complex needs Skills and Competencies To be successful in this role, you will need to possess a range of skills and competencies, including: Excellent communication and interpersonal skills, with the ability to build strong relationships with members, healthcare providers, and community resources Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions Ability to work independently and as part of a team, with a focus on delivering high-quality care and supporting the needs of our members Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities Ability to maintain confidentiality and handle sensitive information with discretion and professionalism Career Growth Opportunities and Learning Benefits At Optum, we are committed to supporting the growth and development of our employees.
