Job Summary We are seeking an experienced and compassionate Remote Patient Monitoring-Home Telehealth (RPM-HT) Case Manager Nurse to join our team in Portland, Oregon. As a key member of our healthcare team, you will play a vital role in coordinating care for Veterans with complex health needs, utilizing health informatics, disease management, and telehealth technologies to improve health outcomes and access to care. Key Responsibilities Assess, plan, implement, and evaluate care for Veterans with complex health needs, utilizing a patient-centered approach. Coordinate care with interdisciplinary teams, including healthcare providers, social workers, and other stakeholders. Provide health education, counseling, and coaching to patients and their families to promote self-management and healthy behaviors. Utilize telehealth technologies to remotely monitor patients' health status, identify potential complications, and intervene promptly to prevent hospitalizations and emergency room visits. Collaborate with community-based providers to ensure seamless transitions of care and to address patients' social determinants of health. Evaluate outcomes of care and participate in quality improvement initiatives to enhance patient care and program effectiveness. Maintain accurate and comprehensive patient records, adhering to organizational and regulatory requirements. Requirements Basic Requirements Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). English language proficiency, with ability to read, write, and communicate effectively in English. Preferred Qualifications Bachelor of Science in Nursing (BSN) degree. Experience in care coordination, case management, or telehealth nursing. 2-3 years of nursing experience, with a focus on complex patient care and population health management.
