Essential Functions and ResponsibilitiesMaintain, monitor, and manage your own patient census across multiple facilities to:Coordinate the implementation of Chronic Care Management (CCM) services amongst chronically ill patients residing in long term care facilitiesCreate individualized Care Plans for patients with chronic conditions. Responsible for updating and tracking monthlyResponsible for monitoring labs, vitals, medications, and testing for patient census.
