Assess and collect data related to patient health status and develop care plans with the care team. Utilize data to meet care coordination needs across settings in conjunction with patients and families. Manage nursing care for individual patients, ensuring accountability and responsibility. Document nursing assessments and interventions comprehensively. Identify patient and family learning needs and educate them using the teach-back method. Provide safe, ethical, and evidence-based patient care that is culturally congruent. Perform telephone, My Chart, and in-person triage for symptom management. Review and act on test results and manage communications requiring RN education and training. Provide patient education through various channels including in-person, telephone, or video. Assist with medication management and complete timely documentation in the electronic medical record. Manage urgent and emergent patient care situations and participate in care coordination activities. Establish standard clinical supply storage and manage medication/immunization inventory. Float to other clinics as needed and serve on relevant committees and task forces. Requirements Bachelor's degree in Nursing or equivalent education, training, and experience. Four years of related experience. Current and unrestricted New York State RN license. BLS certification. Ability to work in an environmentally-controlled indoor setting. Ability to perform sedentary work involving long periods of sitting or standing. Ability to travel between clinical sites as needed. Exposure to blood, bodily fluids, and tissue, with the ability to perform emergency care as needed. Excellent communication skills and ability to work collaboratively with a diverse population. Strong proficiency in computer skills including Electronic Health Systems and Microsoft Office. Nice-to-havesPhysician practice experience preferred. Benefits Competitive salary range of \$98,000 - \$107,000 based on qualifications and experience. Regular work schedule of 40 hours per week, Monday to Friday, 8 AM to 5 PM.
