Job Title: Registered Nurse Case Manager (Wednesday-Sunday Schedule) Location: Charlottesville VA USA 22911-8708Schedule: Wednesday through SundayJob Type: Contract Position Summary:We are seeking a dedicated and experienced Registered Nurse Case Manager to join our team. In this role, you will be responsible for performing comprehensive assessments of high-risk members and coordinating individualized, cost-effective care plans. You will collaborate closely with primary care providers, social workers, and other health care professionals to ensure high-quality care delivery and optimal patient outcomes. Primary Responsibilities: Conduct comprehensive assessments of high-risk members to evaluate clinical, functional, and psycho social needs. Collaborate with primary care providers and the care team to develop and implement individualized care plans. Identify and implement specific interventions to actively engage members and improve compliance with treatment plans. Facilitate access to non-medical supports (e.g., housing, transportation) as needed to support care plan adherence. Monitor and reassess the appropriateness, efficiency, and effectiveness of the care plan over time. Coordinate care transitions between health care settings to ensure continuity of care. Liaise with physicians, social workers, discharge planners, and claims professionals to determine the most appropriate level of care. Engage specialty care resources as needed to resolve complex clinical or social issues. Maintain thorough and accurate documentation of all case management activities, including clinical, functional, and fiscal outcomes. Identify and support opportunities for health promotion and disease prevention. Act swiftly to prevent or mitigate adverse patient outcomes. Serve as a patient advocate, ensuring privacy, dignity, and confidentiality at all times. Qualifications: Associate's Degree in Nursing or a related health care field (Bachelor's Degree preferred) Current and unrestricted Registered Nurse (RN) license in the state of practice Case Management Certification (e.g., CCM, ACM) preferred Strong clinical judgment and the ability to analyze complex medical information Excellent communication, organizational, and problem-solving skills Experience using Homecare Homebase or similar electronic health record (EHR) systems preferred Ability to work a Wednesday through Sunday schedule [ad_2] apply to this job
