Learn about 4C's 4-day, 32-hour work week by clicking below. https://www.youtube.com/watch?v=Aymn0x7fsCQ POSITION SUMMARY: The Care Navigator supports individuals with behavioral health needs by ensuring seamless coordination of care across mental health, substance use, primary care, and community support services. Working within the CCBHC model, the Care Navigator helps clients navigate complex systems, promotes recovery-oriented care, and ensures timely access to services that support whole-person wellness. The Care Navigator does not transport clients as part of their job responsibilities. This position operates under minimal supervision and must exercise tact, judgment, and confidentiality in contact with the public, clients, and families, as well as fellow associates of the Center. Work is very broad in scope and requires a high level of professionalism. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Staff will be responsible for their designated program(s) and/or caseload to coordinate, maintain, and track throughout center wide programs while the case is open. • Understands criteria and purpose of all Center programming. • Assist with internal communication regarding client needs throughout the Center. This includes but not limited to assisting with getting clients appointments scheduled, communicating inpatient admissions to appropriate outpatient providers, ensuring services transfers have been accepted and started. This can occur through email, phone, or virtual meetings. • Maintain service transfer spreadsheets and process service transfers to internal service lines within the EMR within one (1) business day. If client does not meet criteria, communicate back to the requester within one (1) business day. Provide follow-up to internal programs leaders as needed to complete the transfer acceptance or denial. • Run assigned reports on the designated cadence and provide the information to the appropriate leader or designee. Report responsibilities include: No Primary Clinician, No Supervisor, No Primary Program, Clients not seen in 30 days, and Transfer in Progress. • Receives external referral requests from Center providers then complete external referral request in EMR additionally communicates any necessary information to the client via preferred method of contact. External referrals could include but not limited to specialists, housing, employment, education, and social support. This is to be documented in the EMR and should be completed within one (1) business day of the request. • Review med history forms for each client and make recommendations based on their responses. Communicate recommendation to primary provider and to client and document in the EMR. Ensure clients have at least one med history form on file every 12 months. • Organize and facilitate interdisciplinary team meetings and case conferences based on the level of care and specialized population and complexity of the case. • Work across departments/programs to ensure services are scheduled and being delivered according to the client's needs. • Attending all Inpatient daily huddles to coordinate internal appointments post discharge or to make changes to scheduled appointments, and documentation is in the EMR. • Ensure follow-up attempts and appointments have been made/scheduled for clients who have been at our Psychiatric Urgent Care. • Staff will be responsible for meeting referral source updates and needs, for example attending criminal justice meetings or courts, compiling, and sending monthly reports electronically by the 10th of the month, requesting additional units in state systems. • Staff will perform administrative tasks such as checking Medicaid eligibility for designated program clientele to ensure reimbursement for medically necessary services as defined by required assessment tools. Staff will perform these checks on the 1st and 15th of the month, time of a service transfer, or daily if coverage was lost. • Complete financial transfers for any client when made aware of a change in coverage or a service transfer is requested. • Process and complete agency, program, and admin discharges within the EMR within two (2) business days. • Track and report on data collected weekly, monthly, and quarterly periods on the designated spreadsheets. • Assist with outreach and engagement of clients who do not have a Skills Trainer assigned to them. Clients will be entered on a spreadsheet to track and monitor. • Advocate for client rights and culturally competent care. A degree or further education in healthcare or healthcare management preferred but not required. Experience: • Previous experience in Scheduling, Patient Access, Coordination of Care, Registrar or Case Management preferred but not required.
