Conduct comprehensive assessments of residents' health status, identifying changes and reporting concerns to the healthcare team. Develop and implement individualized care plans, collaborating with the interdisciplinary team to ensure that residents' needs are met. Administer medications and treatments as prescribed, monitoring residents for any adverse reactions or side effects. Maintain accurate and detailed records of residents' care, including vital signs, medication administration, and progress notes. Collaborate with registered nurses and other healthcare professionals to coordinate care, address resident concerns, and resolve issues promptly. Participate in ongoing education and training to stay current with best practices in skilled nursing care, enhancing your skills and knowledge. Essential Qualifications Current licensure as a Licensed Practical Nurse (LPN) in the state of New Hampshire. Previous experience working in a skilled nursing facility or similar healthcare setting. Strong clinical skills, with the ability to assess residents' needs and provide appropriate care. Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and the healthcare team. Ability to work 12-hour night shifts, including weekends and holidays, as required. Preferred Qualifications Experience with electronic medical records (EMRs) and other healthcare software. Certifications in relevant areas, such as wound care or medication management. Previous experience working in a team environment, collaborating with interdisciplinary healthcare professionals.
