Key Responsibilities Verify plan of care with the RN prior to each visit, ensuring that patient needs are met and care is coordinated. Collaborate with physicians and RNs frequently to ensure patient needs are being met, and notify them as soon as possible when patient condition changes. Teach patients and caregivers self-care techniques, empowering them to take an active role in their care. Check vital signs and review all medications, performing medication reconciliation at each visit to ensure patient safety. Perform specialized procedures ordered by physicians, such as foley catheter changes, using aseptic technique. Prepare equipment and materials for treatments, maintaining a safe and efficient care environment. Complete documentation timely and accurately, ensuring that patient records are up-to-date and compliant with regulatory requirements. Qualifications and Requirements To be successful as a Home Care Practical Nurse with Amedisys, you will need: One year of clinical LPN experience, demonstrating your ability to provide high-quality patient care. A current LPN license, specific to the state(s) you are assigned to work, ensuring that you are compliant with regulatory requirements. Current CPR certification, demonstrating your commitment to patient safety. A valid driver's license, reliable transportation, and liability insurance, enabling you to travel to patient visits.
